Collate No Case Index is not being triggered #553

Some of the indexes have a NOCASE collating sequence while the columns being indexed use the default BINARY collating function. This causes the indexed not to be triggered/used when performing sqlite queries http://www.sqlite.org/datatype3.html#collation
