[ ] The access of this web page will be governed by a role named Manage Client Form validators.
[ ] Only users with this role will have the ability to add/edit the form validators
[ ] The link to this page should be part of the OpenSRP web Setting menu

Attached is a sketch of what we imagine the page to look like. @rowo will advice further on the UI & UX bits. add_validator_sketch
