Create TBR logo #46

@rowo Can this be closed? Are we going with the "Placeholder" logo from this issue: https://github.com/OpenSRP/opensrp-client-tbr/issues/?

No it cannot be closed. We cannot use the placeholder logo.

Ok then. What's your timeframe for this?

I have no immediate plans to work on this. I was planning on starting a few weeks before final delivery so the logo image and icon can be swapped out, but let me know if that is a problem.

So, depending on your definition of final delivery, I think we're looking at end of Jan 2018 for hand off of the system to IHS for testing.

@alihabib @kwasim can we use "Stop TB" for the app? Another option is just to use just "TB REACH". An icon isn't really necessary though we can use the "stop sign TB" as the app icon.
