QA Testing: Index columns widths have changed across multiple books #924

Widths of index columns have changed with 2-column precal code on QA server. See example below -- note where the second and third columns align left. The new PDF is on the right. This exists in Bio and Physics, and most likely (all) other books. screen shot 2015-02-02 at 12 55 25 pm
