It happens automatically, there's nothing an extension can do about it anyway, so I guess you are a victim of the new UI in Firefox. Both Firefox and Chrome don't pin the icon of new extensions automatically anymore, so the users have to do it manually: https://support.mozilla.org/mk/questions/1408982
Thanks for the link – the addons have always been in Customize Toolbar; I didn't think of right clicking extensions shown in the extensions puzzle piece and think most users will not think of right clicking these if there's not at least a note about this in the Customize toolbar… page.
I'm using this addon to create spoken Wikipedia articles as described here. The problem is I can't add this addon to the addon-toolbar in the top right of Firefox. To turn the style I configured on or off I have to go to the Manage addon page, open the preferences of the addon and there enable the style. It would be much better if there was an addon in the addons toolbar that I can click to see enable or disable a style with just two (maybe 1?) click. Please enable having such an addon icon/button in the Firefox toolbar.
