Add application blacklist reason #341

When an administrator blacklists an application on an OpenWRT node, there should be an optional field where the administrator can explain why the application was blacklisted. This information can then be stored for later reference.

Not sure if this is worth implementing unless we come up with a plan on how to utilize and/or display the stored reasons. Is there sufficient utility to this?
