Document what to do in case of IO errors #47

Please create flow of what to check in case of IO errros on a vdisk.

@jeroenmaelbrancke @jtorreke @matthiasdeblock can one you pick this up and provide me some basic guidelines on what to check and look for?

There is already some documentation provided in the volumedriver docs, why should this also be documented here? https://github.com/openvstorage/volumedriver/blob/dev/docs/restartVolume.md

@kinvaris thanks for the link (can we update the doc as ETCD is no longer the preferred distributed config mgmt system). Also There could be more reasons why you have IO errors (e.g. not policy is violated in case too many disks/nodes are broken)
