Added the Bootstrapping chapter and the Command Execution rules chapters from the docs.operable.io book.

Edits to cog_book.adoc to point it to the new chapters, to allow it to use the cog.css file, and to clarify some section headers.

Also FYI for other document editors, it looks like the include statements need to be separated by a blank line to function properly

I think my pygments-defaults.css gets rewritten/replaced when I run the asciidoctor command to review my updates. It's not doing anything new as far as I can tell but it registers as a new file. Sorry about that.

Mine does the same thing re: pygments-default.css from time to time. No idea why....

I am going to merge + change up the broken "A Tour link..." I think I might be FUBARing things on my side a bit between GitHub Desktop and GitHub. Grr.
