dental exams can occur during inpatient encounters #18

Currently we say that a dental exam is part of a dental visit, and dental visit 
is an outpatient encounter.

I think dental visit should be defined as subclass of health care encounter, 
which would remove this restriction.

"dental exams can occur during inpatient encounters" is correct but rare. But,  
I think it is correct to dissociate from the dental visit. A dental exam could 
also occur during a screening day at a school.
