Ability to produce a report of a form's fields

This issue is about producing a report which would list:

• field names
• field labels
• and more, TBD

The idea is to help with:

• documentation
• mapping fields for PDF, Excel, etc.

Once comments are supported (#3984) those can be part of the report as well. In addition, there is some overlap with #3693.

The report should be available:

• inline in the Form Builder UI
• as a CSV export

Options for the UI:

• Form Settings tab
  • benefit: exists, already has form statistics
  • drawback: the dialog is small for a report, so a lot of scrolling would be needed, unless we open the report in another dialog
• Separate "Form Report" button/icon

I would be nice to also have:

• a printable HTML view
• a PDF download

An HTML view would be printable but it would need to be produced in its own tab.
