When creating a new form or new form data, a lease is not acquired

Steps to reproduce:

• user A creates a new form in Form Builder
• user A saves the form and leaves Form Builder open
• user B (in a different session) opens the form
• user B is able to acquire the lease on the form
• both users A and B can modify the form concurrently

The same issue arises when creating new form data.
