Output PDD format in separate files by year (based on discharge date) #88

Check if you can restrict Synthea to certain hospital dates.

I created an option that will create a directory for the individual run and populate it with CSVs with the PDD date by year.

I also created an option to pre-filter the synthea data by a discharge date range.
