Ability to override default AsciiDoctor secure mode to allow include directive

Sorry it's taking so long, I've been super busy this week and am just now reviewing this PR. I'm making some changes to how the safe-mode option is configured, which is taking a bit of minor refactoring. I should have it all finished and this PR merged/released in the next couple of days.

Please don't apologize :-) That's great news - this was the only thing blocking adoption for us, looking forward to testing it out this week.
