changed module name, v11-only compat, macros fix, new README, demo video #12

changed displayed mod name to just Card Viewer, fixed compatibility to just v11, tried to fix macros/packs, fleshed out README and added demo GIF. added changelog to readme, removed unused demo video files, and added urls for license, readme, bugs, and changelog in the module.json file
