1) Helm-bibtex is not a dependency, bibtex-completion is. These files are distributed as separate packages on MELPA. It's also absolutely possible to download bibtex-completion.el from GitHub without downloading helm-bibtex and have ORB up and running.
2) Installation instructions are Doom-specific. I'd say at most 30% of ORB users use Doom.
3) Org-ref is not a dependency, strictly speaking, in terms of Elisp. Some functionality will be impaired but the package can be used without it.
4) I've already updated the README with more general information before noticing this PR.
