Are you using KeePassX? I don't think the windows KeePass 1.x app supports
custom icons.

If I can add something to this ticket, it's that custom icons do show up for me
using KeePassDroid 1.81, but that the icons are incredibly small compared to
the other icons in the database. I don't know if this a problem with the way I
added the custom icon or a problem with KeePassDroid, but I hope it can be
improved upon.

And for what it's worth, this problem still exists in KeePassDroid 1.84. (I
hadn't realized that there was a slightly newer version out when I posted
above.)

Just would like to add my +1 to this issue.
I use keepassx on linux and mac, its annoying that none of my icons show up on
my android phone as all of them are custom.
Its been nearly a year since this issue was posted, has there been any progress
on getting it fixed?

Confirmed. (Motorola Milestone under 2.2.1 and Samsung Note under 4.0.4)
The custom icons I have set with Keepassx 0.4.3 do not appear. I used PNGs and
Keepassx did not have any problem displaying them.
I thought they were supposed to work...
Is there a special (working) combination of size + format that we're unaware of
?

The issue is still active.
The custom icons you add on keepassX can't be shown on keepassdroid (appears a
black/empty square).
When will be a fix for this?

I upgraded my phone to a Nexus 4 with Android 4.2.2, and my Galaxy Note to
4.1.2. Keepassx updated to the latest release and still no custom icons.
What gives ?!

Well...
I've just installed alpha4 of keepassx 2.0, imported my database, converted it
to v2 and opened it with keepassdroid. Guess what... custom icons do work with
a v2 database.
Now I have to choose between the ability to modify my database on the phone and
the ability to have custom icons.
Everything else just works, so I'm not complaining (too much) :-p
Since I uploaded the file to my Google Drive, I already couldn't modify it on
the phone, so I'll stick with the version 2 database.
