Good question - @UKChris-osm is the expert on this! I feel like NHS could probably be split up into regions similar to how he set up the fire and rescue items..
I had been thinking about this previously. Having the top-level NHS seemed like the simplest starting point, but in terms of organisation / operator there are lower levels that operate each region or specialist service. I was considering if simply "NHS" was better as a brand rather than an operator, as I think people in the UK mostly refer to their local hospital as an NHS hospital.
My only thought is if we went down the road of lower level operators, then this could be a very big list with a lot of geojson targeting, which could be a lot of work, as well as thinking do all the lower level ones have their own WikiData or social media, meaning would logos be readily available - so my thinking was simply going one step lower and having "NHS England/Scotland/Wales/NI" as part of the NSI.
I think health boards (Scotland) or trusts(?) in England are probably the best way to go - similar to Fire and Police - though I wouldn't object to the four country approach, especially as it would otherwise mean a big list, like you say.
Though to be honest, given there are only about 200 hospitals in Scotland and 14 health board areas - the operator tags could probably be manually added to them fairly quickly. After all, it's not like shops etc in NSI where users are going to keep on mapping them so need the brands - once the hospitals are done there's not going to be many changes.
Looking at the NHS Trust list, this could be a big job - and I'm wondering about logo use if a lot of the trusts don't have their own social media? Is there a way of adding a default / parent logo using the NHS logo in such a case?

I'm wondering whether the NHS top level operator should be in NSI, as opposed to NHS England/Scotland/Wales/NI or individual health boards, e.g. NHS Greater Glasgow and Clyde, which actually run the services at a local level?
