Code Cleanup after initial GitHub App Upgrade

Overview

The first few pull requests were a brute force upgrade to GitHub Apps.

Take a second pass to:

• Replace the Gist Deployment Output - #4
• Understand implication of disabling .netrc
• Remove Scopes from OAuth
• Test other Notifiers/Providers
• Update Unit tests
• Validate switch to postgresql DB
