Port Add note id to Anki 2.1 #112

This pull request fixes the obvious Python 2/Qt4 -> Python 3/Qt5 issues. It also adds a second hook to make sure the Note ID field is filled automatically when the note loads. See discussion here: https://anki.tenderapp.com/discussions/add-ons/14498-porting-an-add-on-from-20-to-21

Thanks. I just glanced at this, and it looks like a nice step towards it working for Anki 2.1, so i’ll just pull it now.

And i just moved the file to its own folder, as required, 3129043f. No i can test what you have done.
