Update firmware for OVC5 revision B

Updating the project to account for revision B changes:

• Update USB in Vivado to new transceiver lanes
• Add SMBUS for USB7216C initialization

Still in progress:

• Cross compiling the software to initialize USB7216C
• Initializing the second USB interface at startup

The changes so far seem necessary and good to me; OK if we merge now and continue improving later?
