Opening and amending of Closed forms #168

I understand that closed forms can be opened in Admin mode where missing data can be amended or data identified as incorrect can be adjusted.

@AllyBradley, how do you propose we handle this? I see various scenario's that might work - Forms are opened when missing values and / or queries regarding data are identified and (identifying those cases with missing data or where queries are needed is another Issue that I will file

a. the local investigator or delegate is asked to complete for Follow-up forms and transplantation forms as they will have most of the data (except machine data). One caveat here is that local investigators cannot know the treatment arm so this complicates queries on transplantation forms as when a transplantation form is openend the treatment arm of the kidney can be derived.

b. procurement forms are completed by TTs, however, there is quite a big turnover in TTs and - once the procurement is over - they do not have access to Eurotransplant databases to complete missing or queried donor data. It is possible that local investigators have those data but it is also likely that some of the data that might be queried will not be known by the local investigator and needs queries to Eurotransplant. The National Investigator would be in a position to contact the people that can answer a specific query and it would be good if they are allowed to amend the data upon receiving the additional information.

This also links to #167 - the lack of any functionality associated with Local, National, or Central Investigators means this is new work to enhance the data collection.

In lieu of a detailed flowchart showing how this process flows, we'll simplify to assuming that data first entered is done by TT's as planned, and this is the next stage of data entry. Both scenarios above are likely, and both rely on Investigator roles and functionality to be able to proceed.

I'll refer to #167 for discussion over what the Investigator roles require. Once they're established, we can then work out the mechanics of how forms are re-opened, or only closed to TTs.

Two changes here proposed to deal with this:

This only addresses the Procurement and Transplant forms though. There are less rigid processes (read: almost no process) on FU and SAE forms, so currently not applicable to them for the open/closed state.

Also doesn't address anything related to Investigator level access.

Additionally, consider allowing Technician's to see a list of their closed cases, to make it easier for them to request re-opening for "accidentally closed" forms.

TT's can now see their closed cases

Co-ordinators can now go and change the status of forms from closed to open again for when TT's make mistakes, so skipping the extra button being added.
