Two Staff Persons with the same email address

SP with ids 291 and 292 both have the same email address, but different usernames...

@AllyBradley has held her hand up to this, and a few other admin errors with user account creation, so she's going to take it on herself to fix it

To change:

• All usernames not following the naming convention set out from the start (see note below)
• Staff Person 292 (Rixt Menger rixt.menger) who has the wrong email address in their profile ...and anything else I discover!

You will need the following information: 

- First Names
- Last Names
- Email address
- Location Based at
and optionally, telephone number

You will generate the following information:

- Username: first initials + surname e.g. smertens, abradley, cmarshall, jfkennedy. If we have more than one person with with the same username (and we’ve already accounted for their first/middle names), then I advise we add a single number to the end, e.g. smertens, smertens1, smertens2, smertens3, etc
- Password: Suggest you use one line of the following per user: https://www.random.org/strings/?num=100&len=12&digits=on&upperalpha=on&loweralpha=on&unique=on&format=html&rnd=new

Password rules are presently quite liberal, but I will tighten up on this in the future. So far:
- Minimum 12 characters
- Must contain at least one non-number
- Must not be in the list of 1000 most common passwords
- Must not relate to their username, first or last names, or email address

The steps overall are:
1) Goto the Admin interface - https://cope.nds.ox.ac.uk/en-gb/admin/ 
2) Create a User record (two stage process, first you set username and password, then save, then enter first and last names, along with adding them to the Technician group)
3) Create the corresponding Staff Person record
— rinse and repeat.

I will remove any duplicate records affected here (including the SP records with duplicate email addresses) as part of Issue #180, however @AllyBradley still gets the job of changing the usernames for the incorrect ones and notifying those users.

Who can create usernames/passwords for staf personel (transplant technicians, local investigator/study nurse)? Before I was able to create those as well but at the moment I can't.

Has been cleaned up manually. Staff records are now handled via a new front end process.

This issue has reared its head again following #337 and activities over the past year. There are now 30+ user accounts for Fenna vdl, 8+ for V. Huuren, and 20+ for Jereon DJ. Not yet clear if data cleanup to remedy this is prudent, but linking to the current issue that has highlighted this for reference.
