WP451021: problem with closing form #205

@marshalc Could you have a look at donor form WP451021. The TT wants to close and save the form, but the database says that there is an error in the form. It doesn't say which error there is. I took a look at the form, but I can't find where the possible error is. Could you have a look and let us know what needs to be corrected in order to close the form?

Was the TT Marie-France Jilderda?

If so, then I suspect the answer is simple and linked to Issue #206

The TT was Paco Deckers.

donor form errors:

date_admitted_to_itu:
- 2107-02-03 is not between 1900 and 2050
- 2107-02-03 is in the future

This error was not visible because the answer to Were they admitted to ITU had been changed back to No (or more likely, not saved as Yes in the first place - though I can't yet work out the logic of how this date was saved incorrectly in the first place).

Can be fixed on the live site by someone ( @AllyBradley ? @mertenssarah ?) who has the correct data.

@mertenssarah Sarah can you check with the TT if the donor was admitted to ITU and if so the date?
