Re-open transplantation form WP452010R #213

Yesterday I got a message from a TT that form WP452010R got closed accidentally after not adding all information. The TT saved the form once (didn't pushed save 2 times) and got a error message: He had to put in that the kidney was not reallocated. He already did that. He saved again but than the form got closed.

@marshalc : Can this form be re-opened?

The transplantation centre is Universitair medisch centrum groningen. Maybe he picked the wrong transplantation centre?

Data clean up to be done post 0.8.0 release

@AllyBradley - one for the data clean up? Form is already editable by co-ordinators (who can re-open by unclosing the form).

Will remove this from the release milestones as not a development task.

The only mandatory item missing from this form is the time machine perfusion stopped. According to Ina's document "Mandatory Desirable Data" this data will be obtained from KidneyAssist machine files.
