WP446004: right kidney randomised as: not set #275

After randomising WP446004 was the right kidney randomised to "not set". How is this possible?

Unclear at this stage, so am investigating. The next randomisation has completed and recorded correctly for L&R. This randomisation has only recorded for L, not R.

Data has been corrected for this record, however will do further testing to see if we can reproduce this unique event.

7 test randomisations all preceded as expected (including one that results in ID WP446004.

Now going to do a scan to see if there have been any previous occurrences of this phenomenon.

No other instances were found. Closing until we can find a reproducible example.
