Add scanned documents to admin only view of database forms

To help tally paper records against digital ones, we need a two stage process, and some help:

• Paper records once entered need to be labelled (physically) with a system ID that confirms their:
  • Link to a given digital record (so system ID number)
  • Date of upload
  • Version of the digital record that matches the paper version (this will be tricky to do retrospectively)

Those paper forms then need imaging, and linking to the digital record so they can be viewed alongside the digital form for data entry / comparison.

This issue is likely moot now the trial is closed to new applicants and data cleanup is mostly concluded.
