Urine and Perfusate Sample collection reports are missing #290

Both are coming up with missing templates in the 0.8.2 release. Likely needs to be bundled into the related Biobank work issues that are going to be the focus of 0.8.3.

Initial templates added. Now need to review that the data is sufficient

Additional work needed on all four reports:

Summary data showing counts of matches, etc. Partly done for tissue, but not for the others.
