Data "removal" for non-consented patients #328

Once the final check up of what patients have and have not consented to the trial (the delay was mostly due to Dutch retrospective consent processes), we have to do a data purge for non-consented data, and ensure this trickles down through all aspects, so we need a report to state that this data was removed as there won't be a way to show this in the live data otherwise.

As per discussion in #327, the process for removal is similar to the inactive conversation, but with the addition of looking for related records in:

Reliant on others to deal with the paper form and misc (spreadsheet) data to be also destroyed.

50 Non-consented and 7 Unknown recipients have been identified in the live data using the new view added in the commit referenced (v0.9.2 release). Awaiting feedback from @mertenssarah via email about what to do with these records.

I have scheduled this for the agenda of our next call.

We would need to decide what to do with these. Also, before removing any data, we need to be 100% sure. Keeping donor data is fine
