Update file upload behavior and UI #9

Progress

[x] Clean up file upload (read) state
[x] Fix error uploads block mockup generation
[x] UI update on file list item
[ ] UI update on Upload position (Because of this, suggest handle with MUP-49 altogether)
[x] Allow remove file list item
