[PROTOCOL] Scrape IDSA protocol lists (clinical) #108

Note: this is going to be a bit of a rabbit hole, since each link opens up another resource to scrape / standardize, and may induce duplication with other resources already scraped.

Where possible, add in Protocol Categories and Protocol Settings (probably all/mostly clinical?)
