Concepts and their concept_ids, at least in UMLS, refer to unique ideas that multiple codes might point too. In GDM we are not doing the UMLS-style concepts and we should consider avoiding this terminology if we can. Options might include "code_id", "atom_id", "term_id" or other words that are less loaded or more accurate.
