Remove GDM Vocab Tables #127

We are going to start using OHDSI's vocabulary tables directly, rather than deriving our own.

We should remove our custom vocabulary tables from the GDM specification and change all foreign key references to "concepts" to "concept" and "vocabularies" to "vocabulary".

I'm not sure where in the README we should link to the OHDSI Vocabulary Tables
