Changed the lines table to the provenances table which holds linkages for both lines and claims. clincial_codes now has only a link to the provenances table. The claims table was replaced by the collections table which groups provenances.
Moved the clinical_concept_id's in the exposures and details tables to the clincial_codes table and made those table's ancillary tables to the clincial_codes table. The details table was renamed measurement_details and the exposures table was renamed drug_exposure_details.
