Change provenances table to contexts table

This table now more reflects services than just data provenance.

• Adds an address_id field to associated a particular service instead of linking to just a particular provider who then has an address associated with him.
• Adds facility_id and type to table.
• Adds service_specialty_type_id which is the specialty associated with a particular service. This allows multiple providers to have different specialties associated with them.
• Change collections_providers to contexts_provider
