Updates from 07 2016 meeting #72

Updates CDM from in office meeting on 07/08/2016 with the following changes:

people -> patients
providers -> practitioners
practitioners.birth_date datatype to date
add collection_id to clinical_codes
change file_type and information_type to xxx_type_concept_id
remove pos_concept_id from collections table
change all variable names that use a concept id to look like xxx_concept_id
