I'm facing the same issue on macOS Big Sur 11.1 arm64. Version of Telegram I use is 7.3 (211379) AppStore.

I've found a problem, also with the update 7.4.0, using videocalls on Telegram on macOS Big Sur 11.1 and MacBook Pro with M1 chip. Starting a videochat (or receiving one) the webcam starts working great and, after few seconds, the image stucks. Also disable and re-enable it shows the previous frame after the stuck.
After closing the videocall, green led stills on. It's necessary force the closing of Telegram and re-open it. I've tried also with an external webcam and the problem is the same.
Configuration Operating system: macOS Big Sur 11.1 MacBook Pro M1
Version of Telegram Desktop: 7.4
Used theme Night mode

I had 7.3 then downloading this file https://t.me/macos_stable_updates_files/82 and reinstalling Telegram (move the file to Applications and replace the old one). Video call has no issue now, just tested it again.
Yes, the 7.3 was from App Store.

I was wrong. The problem still occurs on mine but somehow it works fine with no issue in full screen mode.
Do you have crash or it’s just freezing?

I got the same issue when I open Telegram not in full screen mode. I thought it was fixed on 7.4 LOL
