Restructure repos

Major refactor of the repository, to allow code to run and generate the dataset.

• Renamed main script and shared module.
• Added a requirements file properly defined.
• Created a folder of required inputs (with country harmonization files).
• Moved data files to a new folder.
• Moved all temporary data to a temp folder (ignored).
• Fixed all paths to be able to read and write.
• Fixed some other issues.

With all these changes, I checked that the code works (it has to be run first for 2011 and then for 2017), and the file is also properly copied to S3.
