Split chapter 5 #11

There is weird situation where we use some functions before they are defined #9 so I would propose to restrict chapter 5 to simple cohorts and add an extra chapter later on where you add inclusion and exclusion criteria to the cohorts (and include cohortConstructor functions there), would you be happy with that @edward-burn ?
