And the mutant enderman uses its passive AOE attack when it teleports, and a mob is damaged by this age attack, the resulting crash occurs:

You can trigger the passive AOE attack by punching it in creative mode (may take a few tries). Make sure there's at least one mob next to the Mutant Enderman when you do.
Sorry, I know this is a really specific situation, but I'm not certain how else this crash occurs, as this is the only situation I've seen it occur in so far.
Also notable, any mobs hit with the AOE attack will still attack the Mutant Enderman when the world is rejoined after the crash.
