Update and refactor to new formats to use correct validations and forms

@jeffpatzer let me know if this is stale, or if the Okta SAML wizard still needs updating with the new components.

I haven't updated it quite yet. It works, but doesn't use the other forms (which display errors correctly). It's the next thing on the list though and I was planning on finishing it today.
