Remove legacy version management #213

Issue #35 implemented a new versioning system for Herbert and Horace.

The legacy SVN / $Revision in headers can be deleted and existing (out-of-date) revision information removed from the headers.

There are ~1000 files containing $Revision:: strings in the file headers to update.

The commits removing the header comments should be made without any other code changes to keep the PR manageable.
