If you give a rat an item, and then duplicate it with the void altar, the item also gets duplicated. I would assume this also works with zombies holding items, and so on. Perhaps the void altar could be altered so it just spawns new entities of that type instead of copying them perfectly, or alternatively the void charm could be made to not work on entities that have sub-tags with item damage values somewhere in their NBT.
