OTU notes and/or comments #148

For OTU entry, we need to add a free text "Notes" field to the bottom of the optional fields. This is for the enterer to make notes about the OTU.

We still hope for the ability to add Comments to OTUs so that users can converse about OTUs in a trackable manner.

@NoisyFlowers Probably should close this one. Seems like it was also added to Issue #150.
