slime-break-on-exception: invalid docstring and usage of prefix argument

Hi Jürgen,

I think the docs string is still wrong - shouldn't it be something like 'With prefix argument FLAG, do not break on exception"?

If you could fix that and squash everything into a single commit, that would be great.
