the Android camera encountered a problem, You may need to restart the device

What steps will reproduce the problem?
1. From Eclipse, run the program
2. On the device (running 4.0.3 Asus Tablet), hit, Scan Code
3. Error shows up

What is the expected output? What do you see instead?
The camera is expected to show and allow me to scan, but the error in the title 
comes up.

What version of the product are you using? On what operating system?
Asus TF101 running 4.0.3

Please provide any additional information below.
The app runs fine in the simulator
