default.pgm: add a notice about pgm.controller.offset config setting -- #50 #53

Let's add a warning about pgm.controller.offset configuration option ! (#50)

I'll rephrase this:

Note that by default the program-number is in the range 1..128.
The first program (display-number: "1") is loaded by sending a
MIDI patch-change to activate program "0".
This can be changed setting pgm.controller.offset=0 in configuration.

Oops yes indeed. I've to stop doing 4 things at the same time ;-) I really wanted to finish this before going to sleep.
