I too am having this same issue, one of my pawns got bit on the ear, and during the countdown where it says it will get infected in X hours I tried to do an amputation on said ear, with glitterworld medicine and a pawn with 18 medical skill she kept failing the surgery, the progress bar gets to the end, then there is a spark animation as if she failed, but no messages pop up about any failures and the injured pawn gets absolutely no damage due to the failures either. Also the body part doesn't get removed. I tried the amputation 17 times in a row with absolutely no success.
Another issue I'm having which I'm not sure if it's related to this same issue is when I get those random transport pod crashes, the survivor is never injured or needs rescuing. The pod crashes, pops open and the survivor just walks off the map with absolutely no injuries. Even under their health tab it reports no injuries and they just run off the map.
Upon completing a "removing bodypart" operation, it plays the visual and sound effect for a failed surgery but doesn't show the usual "ridiculous/catastrophic failure" message. No damage is applied to the patient and no body parts are removed from the person.
Tested with only Core, CE and ZL loaded
