Added chain selection to form

Updated interface to have the chain dropdown inside the form:

We can modify this to add new chains like @helloitsbirdo suggested.

Also, updated all dependencies to latest

I added it as a direct link, when you pick the chain, there will be also the ability to move to different sites.
