The chainHead_follow subscription is now producing the responses of other chainHead methods:

chainHead_storage, chainHead_call, chainHead_body are now plain methods
generate a unique operation ID to identify the response in the chainHead_follow events
propagate the response with the operation ID to the chainHead_follow subscription
