Case Form - Edits to "Open to All or Limited to Some" Values #823

4th value added:

  1. Open to All
  2. Open to All With Special Effort to Recruit Some Groups (e.g. community organizing to recruit low-income earners)
  3. Limited to Only Some Groups or Individuals
  4. Both (i.e. some aspects are open to all and others are limited to some)

NOTE This changes the behaviour of the "Recruitment Method for Limited Subset of Population" field - see issue #824 - AND the behaviour of the "Targeted Participants" field - see issue #826
